When you come into our offices, we have a couple of forms you will need to fill out. One of them is a basic information sheet with things like your contact information, how we can get in touch with you, that kind of thing. We have this form in our office, but you might want to save time and fill it in at home, print it out on your own computer, and bring it with you.
